The Diocese of Superior, Inc. offers three health plan options to eligible employees and their eligible
dependents. A Summary of Benefits Coverage is available to eligible employees by clicking on the
Employees Only link to the right. To obtain the password for the Employees Only Resources page
contact Bernie Kroseman in the Insurance Administration Office.
The Summary of Benefits Coverage for the three health plan options provide information on
deductibles, covered services, coinsurance, copays, out-of-pockets expenses and limits.
IMPORTANT: When receiving care, present your identification (ID) card to the provider who is rendering the services. If you have questions about your coverage, please contact the Claims Administrator at the address or telephone numbers listed below.
Eligible employees of the Diocese of Superior are generally assigned one of the following networks.
The Employer Provider Network, Inc (EPNI) is for employees/families who live in Minnesota or bordering Wisconsin counties.
HealthEOS is for eligible employees/families who live in Wisconsin counties which do not border Minnesota.
The Diocese of Superior is exempt from the requirement to cover contraceptive benefits for employees who are eligible for health insurance. For more information about the Catholic Church's teaching on contraception click here.
The Claims Administrator’s customer service staff is available to answer your questions about your coverage and direct your calls for prior authorization, preadmission notification, preadmission certification, and emergency admission notification. Customer service staff will provide interpreter services to assist you if needed. This includes spoken language and hearing interpreters.
