The Diocese of Superior offers full-time and part-time C employees the opportunity to participate in the Flexible Benefit Cafeteria Plan (Flex Plan). The new plan year is 09/01/16 through 08/31/17. To participate, employees must enroll each plan year. Employees have two options for enrollment: 1) online enrollment; 2) submit a paper enrollment form to the address or fax number on the form.

The deadline to enroll either online or by paper form is August 16, 2016. Failure to meet the
deadline means waiting until the next open enrollment period in 2017 to participate in the benefit.
